UFT (Unified Feedback Tracker), the Uttar Pradesh state government’s health department, is designed to support data collection, supervision, and monitoring of facilities/institutions state officials through surveys, checklists, forms, and assessments. The application is free of cost and not intended for commercial use. We take a consent from users for using the application and do not automatically capture any specific personal information from users (such as name, phone number, or email address) that allows this application to identify an individual when they visit the App. Users can generally use the App without revealing personal information unless they choose to provide it. This page is used to inform users regarding our policies with the collection, use, and disclosure of Personal Information for anyone who decides to use our Service. If user agrees to use this application, they agree to the collection and use of information in accordance with this policy. The application may collect GPS location data only when the user actively uses the geo-location feature. The location is captured at the time of visit when the user clicks on the geo-location option in the application.

By using this application, users agree to provide camera access to capture images (such as facility/institution photographs). These images may be used by authorized officials to review and monitor field conditions.

We want to inform you that whenever you use our Service, in a case of an error in the app we collect data and information (through third party products) on your phone called Log Data.
